Select the primary author from the following drop down list. If the primary author is not in the list, please go to the Colleague Database to add the person.

For secondary authors, please try to select authors from the colleague drop-down list first. If they are not in the list, select Other, and input their name directly in the [Last Name], [First Name], [Middle Initial] format.

If the organization is not in this list, please go to the Colleague Database to add it.

Please put each keyword on a separate line. You do not have to put multi-word keywords in apostrophes or quotation marks.
